What's the Best Treatment for Pinched Nerve?

Monday, 24 November 2014 02:32 PM EST

Question: I have lumbar spinal stenosis, a large synovial cyst results in a pinched nerve, and the pain in my back left side across buttocks down leg is unendurable. My orthopedic surgeon recommends spinal fusion surgery (a spinal plug lumbar fusion and pedicel screw instrumentation). I also have bone spurs, a bulging disc, and arthritis. But I am worried because I am 86 years old (and female). Should I get a second opinion?

Dr. Hibberd's answer:

I share your concerns. Surgery at age 86 involves more risks than surgery at 60. Often less invasive options are available. Perhaps the synovial cyst can be accessed by a minimally invasive technique, and another choice for correction of your lumbar spinal stenosis can be offered.

You owe it to yourself to seek out at least a second and possibly a third opinion.

Be sure to discuss all your options with your primary care physician also. Specialists who operate on the lower back include orthopedic surgeons and neurosurgeons specializing in spinal surgery. Ask and seek consultation to see whether you may be a candidate for laser spine surgery also.

Sometimes a consultation with a medical therapy specialist called a physiatrist is advised, especially when the risks of surgery are being considered.
