Question: My wife's been having pain in her right shoulder joint for a long time. It gets worse at night when she goes to bed and pain medicine doesn’t help much.

Right shoulder pain worse at night often is referred from the neck. Pressure or inflammation of the nerves and nerve roots of the cervical spine is common with disc disease, injury, and arthritis. This pain often responds to medication.
Shoulder joint pain is most likely from osteoarthritis or bursitis of the shoulder, or even an osteoporotic fracture or rotator cuff strain.
I’d recommend an exam with her doctor and an X-ray, or possibly even an MRI of her cervical spine and shoulder.
