The Equifax worker who failed to install a critical security patch left the company after a massive data breach, former Equifax CEO Richard Smith said Wednesday.

Smith appeared before the Senate Judiciary Subcommittee on Privacy, Technology and the Law and told lawmakers the person whose inaction contributed to the breach was one of several who left the company after the breach was made public, The Hill reported.

Four people left Equifax after the breach was revealed last month, including Smith, the chief information officer, and the chief security officer. The employee who did not install the patch also departed, Smith confirmed Wednesday.

Smith appeared before the Digital Commerce and Consumer Protection subcommittee of the House Energy and Commerce committee Tuesday and said a single employee did not heed warnings to install a security patch that could have prevented the breach.
