The Air Force says its failure to properly report criminal information to civilian authorities about Texas church shooter Devin Kelley was not an isolated incident, Military.com reported.

In a statement, Air Force spokeswoman Ann Stefanek said based on initial findings in an investigation, the Air Force Inspector General confirmed authorities at Holloman Air Force Base "did not report required information" on the former airman, the outlet reported.

"The review also found the error in the Kelley case was not an isolated incident and similar reporting lapses occurred at other locations," Stefanek said in a statement, Military.com reported. "Although policies and procedures requiring reporting were in place, training and compliance measures were lacking."

She did not elaborate, but said the service "has reported and corrected several dozen records" amid its ordered review of some 60,000 records involving airman committing "serious offenses" since 2002, Military.com reported.

Earlier this month, the Air Force said it is likely Kelley's violent domestic violence offense was not entered into the FBI's National Crime Information Center database.

And because the agency was not aware of his criminal record, Kelley was able to buy an assault rifle-style weapon used in the mass shooting Nov. 5, described as the deadliest ever to occur in Texas.
