A man diving to inspect a million gallon water tank in Braintree, Massachusetts, died Thursday after a problem with his air tank developed.

The inspection began at the Lincoln Heights water tower Thursday morning, when around 10 a.m. the man radioed a spotter on top of the tower that he was losing air, the Boston Herald reported.

Although the spotter went into the frigid water to try to save the diver, he quickly weakened in the cold, windy conditions and had to be rescued himself by firefighters who were called to the scene. 

The spotter was taken to the hospital and treated for hypothermia, according to the Boston Herald. Firefighters then decided to drain the tank to recover the diver’s body in a “respectful manner,” according to Braintree Mayor Joseph C Sullivan.

The process of draining the tank took 12 to 14 hours, and the body was recovered overnight, according to Braintree Patch. The draining and refilling did not affect Braintree’s water supply.

Although the investigation is still ongoing, the cause of the diver’s death was thought to be equipment failure. Frigid conditions and 50- to 60-mph wind gusts made both the inspection process and the rescue difficult, The Boston Globe reported.

The diver worked for T.K. Potable Diving of Texas, which is a subcontractor of Pittsburg Tank & Tower Co. of Kentucky, which was hired by Braintree to inspect the five towers that supply the town’s water, The Boston Globe noted.

“It’s a tragic day,” Sullivan said at the scene, The Boston Globe reported. “It’s a difficult day for us. That individual was providing valuable service to our community. We also saw the best of our fire department and others who stepped up and took the time to save a life.”
