Can Testing Diagnose Hyperparathyroidism?

Question: Is there test for hyperparathyroidism? Would this be something that a primary care physician does or a specialist?

Dr. Hibberd's answer:

All medical doctors should be able to diagnose hyperparathyroidism (overactivity of the parathyroid gland). Treatment is usually surgery. Hyperparathyroidism is caused by a tumor of one or more of the parathyroid glands.

Parathyroid hormone levels (PTH) are measured by a simple blood test. If the calcium level in your blood is elevated or has been elevated on occasion, be sure you have your doctor order a PTH test for you.

Here are the essentials you need to understand:

• PTH is made in our parathyroid glands (we have four). Parathyroid hormone controls blood calcium levels.
• If the blood calcium is too high, it can be associated with a low parathyroid hormone level (as the gland secretion is normally shut down by high calcium levels).
• If your blood calcium is too high and the parathyroid hormone level is high, the cause is usually a tumor of a parathyroid gland.
• If your blood calcium is normal sometimes and elevated sometimes with a normal or elevated PTH hormone, you may still have hyperparathyroidism.
• Hyperparathyroidism can only be ruled out when PTH levels are low in the face of elevated calcium levels.
• Hyperparathyroidism treatment should be directed by your primary care physician in consultation with a surgeon and an endocrinologist.
