NEW YORK -- A New York Police Department report says nearly 1 in 6 undercover officers has been mistaken for a suspect by fellow officers — and come face-to-face with a loaded gun.

The report comes after an off-duty officer in plainclothes was killed by another officer. The off-duty officer had drawn his gun and was chasing a man who had broken into his car when the other officer mistook him for an armed suspect and shot him. Gov. David Paterson ordered the report after the May 28 killing.

Police Commissioner Raymond Kelly detailed the information in a plan to avoid such shootings.

Cases in which undercover officers are actually fired on and killed are very rare.
