The New York Post reports
that Midtown Manhattan's Guy and Gillard restaurant delivered the $6 salad to theatrical agent Robin Sandusky's office in the Chelsea neighborhood.
Once she began eating, she looked down in horror as she realized what was on her fork.
"I turned it over, and I could see its eye," she told NBC 4 New York
"It was the craziest thing. After a few bites, I look down at my fork, and think, 'Oh, is that a piece of asparagus?' And then I saw that it had eyes, and an arm," she told the New York Post.
Sandusky provided several media outlets with a picture she snapped of the alleged lizard, which quickly received hundreds of retweets across Twitter.
Not the first salad she'd ordered from the restaurant, she was surprised at the incident.
"I order from them all the time," she said. "The same kind of salad at least every day for the last two weeks, and it’s been fine."
The restaurant received an "A" health rating on its most recent inspection, and the manager was just as surprised as his customer when she returned the salad.
"Honestly, I've never heard of something like this. I've never seen a lizard in New York!" he exclaimed.
He apologized to Sandusky, and gave her a full refund. Unfortunately, he said, he was not around when the delivery person retrieved the salad.
Managing partner Jason Jefferies said they will investigate the incident.
In December, a New York City Wall Street Journal employee found a dead frog in her salad
